These reports for are available on the eTech Ohio website through the PD Contact's Hall Pass.

Please follow the directions below to access and complete these reports:

1. Access the Conclusion requirements for your PD Grant FY05 application
   • Go to http://www.etech.ohio.gov/go/myapplications.
   • Login with your Hall Pass account.
   • Select PD Grants FY05 from your list of applications.
   • Select the "Conclusion" tab.
2. Complete the "Final PD Project Report: Impact" requirement.
   • Select "View/Submit" from the Action column next to this requirement.
   • Complete all questions.
   • Click the "Submit" button on the bottom of the page to complete this requirement.
   • Note any alerts that may appear after clicking "Submit," correct any errors and re-submit.
3. Complete the "Final PD Report: Financial" requirement.
   • Select "View/Submit" from the Action column next to this requirement.
   • Complete all items.
   • Click the "Submit" button on the bottom of the page to complete this requirement.
   • Note any alerts that may appear after clicking "Submit," correct any errors and re-submit.
4. Complete the FER Signature Page
   The Action column will read "Disabled," until the "Final PD Report: Financial" requirement has been approved. When the "Final PD Report: Financial" requirement is approved, you will receive an email informing you that the link for the FER Signature Page has been activated. To complete the "FER Signature Page" requirement.
   • Select "View/Print" from the Action column next to this requirement.
   • Print this form.
   • Obtain the appropriate signatures from the treasurer (CFO) and superintendent (CEO) and return the original, completed form to the address listed.
