Please note that all information shared regarding the BETA Teacher survey is completely anonymous.

BETA08-09 Teacher Survey Instructions

   1. Click the "Select Building" link to choose your building.
      
      
   2. Type the name of your building in the "Organization Name" field and click "Search."
      
      
   3. Select your building from the list of matching names. If your building does not appear in the list, click the "Update Search Parameters" link in the upper left of the results window. Try using a more general form of your building's name (e.g., "Crestview" rather than "Crestview Middle School"). If there are multiple buildings listed, please select the one from your county.

   4. Once you select your building, you will be taken to the Survey Response Entry page. This page contains the entire survey. Please provide a response for each question unless it is marked "optional."
      
      
   5. Questions with a "?" next to them have additional clarification text available. Click on the question icon to display this additional information. When you have responded to all of the required questions on the page, click the "Submit" button at the bottom of the page.

   6. When you click the "Submit" button at the bottom of the page, your responses are checked by the server. If the server finds a blank question that requires a response, you will see this message at the top of your screen. Scroll down the page and look for questions that are flagged as shown in item "G" below.
      
      
   7. The "Question requires a response" label will appear below any item(s) that the server finds need to be answered. Select a response from the available choices for each question and click the "Submit" button again.
